The Oasis Center is a non-profit medical clinic providing affordable family medical services. The Oasis Center Sliding Fee Discount Program helps uninsured or underinsured patients to receive medical services. All patients may apply for a sliding fee discount, discount is based on family size and income. No patient is turned away from services due to inability to pay.

How to apply:
To be eligible for the Sliding Fee Discount Program an individual is required to:
- Complete Program Application
- Provide proof of household income or financial assistance – these include tax return, last 3 paystubs, signed letter from employer, or self-declaration of income.
- Provide picture identification
Sliding Fee Discount Rates:
All payments are due at the time of the service. If a patient is unable to pay the balance at the time of the services a payment plan may be established, and remaining charges will be billed to the patient.
Nominal Fee- For Scale “A” visits the cost is $25
